EXTENDED HEALTH SERVICES: Our services are not covered by OHIP however, many of our health care professional services may be covered by your extended health insurance carrier. We provide all necessary documentation for you to obtain reimbursement from your provider.
EXTENDED HEALTH INSURANCE: Insurance companies cannot be billed directly. Our clinic is not responsible for any balances that your insurance company will not cover. It is the patient's responsibility to understand their extended health care policy. Patients are responsible for the provision of payment after each visit. 24 HOUR CANCELLATION POLICY: Your appointment time is reserved just for you. A late cancellation or missed visit leaves a gap in the practitioner's day that could have been filled by another patient. As such, we require 24 hours’ notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours’ notice, or miss their appointment, will be charged a cancellation fee. We thank you for respecting our time.
